Student Complaint Form (Official Process)

Start Your Complaint

Use this form to submit a formal complaint, concern, or suggestion to UAEACC. This form is the first step in the College's official complaint process.

⚠️ Before You Submit (Important)

All complaints must begin through UAEACC's internal process. External review (ADHE/SARA) occurs only after the institutional process is complete and focuses on whether proper procedures were followed.

✅ What This Form Is Used For

Use this form to:

• Report concerns about services, processes, or interactions
• Submit formal complaints
• Provide suggestions or feedback

⚠️ Do NOT Use This Form For

Some concerns must follow a different process:

• Grade disputes → Use the Academic Grievance Process
• Discrimination, harassment, or Title IX concerns → Use Title IX / Civil Rights reporting
• Student conduct issues → Use Student Conduct procedures

✅ What to Expect After You Submit

• Your complaint will be reviewed by the Office of the Vice Chancellor for Student Services
• You will be contacted within 3-5 business days
• The College will work with you to help address and resolve your concern
